Community Trust Bank $100 drawing eStatement Promotion Official Rules. No Purchase Necessary to Enter or Win
Open Only to Legal United States Citizens, 18 Years of Age or Older
Prize: Community Trust Bank will give away $100 (the “Prize”) during the promotion period (defined below) of its “eStatement Promotion” in accordance with these Official Rules.
Entry Period: The promotion begins at 12:01 a.m. Central Standard Time on June 1, 2013, and ends on August 31, 2013, at 11:59 p.m. Central Standard Time. This is known from here forth and the “Promotion Period.”How to Enter: Subject to the “Conditions of Entry” below, there are two (3) ways to enter. You can enter this Promotion online or by regular mail.
- Enroll online in eStatement delivery from Community Trust Bank: to enroll for eStatement delivery in connection with your Community Trust Bank checking, savings or money market account (each, an “Eligible Account”), click on the eStatement link (https://www.ctbonline.com/pages/estatements) under the “Online Banking” tab on the Community Trust Bank homepage (ctbonline.com).
- Mail Entry: On a plain 3”x5” card, legibly handprint or type your name, complete street address, city, state, zip code, home telephone number, and email address, and mail the card to: Community Trust Bank eStatement Promotion, 1511 North Trenton Street, Ruston, LA ZIP. Mailed entries must be postmarked and received by August 31, 2013. Only one entry per envelope is permitted.
- Already enrolled: If you are a customer and are already enrolled in Community Trust Bank’s eStatement delivery, you will be automatically entered into the Prize drawing.
Limit one entry per eStatement enrollment. Entrants who used mail entry are limited to one entry per household. All entries are the exclusive property of Community Trust bank and will not be returned.
Odds of Winning: Odds of winning will vary based on the number of eligible entries received.
Consent to Publicity: Except where prohibited, registration for this Promotion constitutes each entrant’s consent to Community Trust Bank’s use of his or her name, likeness, voice, opinions, biographical information (including without limitation, hometown and state) for promotional purposes in any media without further payment or consideration.
Selection and Notification of Prize Winner: Subject to these Official Rules, a designated Community Trust Bank official will select the Prize winner by random drawing from along all eligible entrants on or around September 5, 2013. The prize will be awarded no later than August 31, 2013. The prize winner does not have to be present at the time of the drawing.
On or about forty-eight (48) hours after the determination of the Prize winner, the Prize winner will be notified by email, phone, or regular mail. The Prize winner must claim his or her prize within ten (10) business days after the date of the Community Trust Bank’s first notification attempt, which shall be the date notification is sent if by email or regular mail, and the date the call was placed to the winner, if by phone. The Prize winner must claim the Prize at Community Trust Bank’s headquarters at 1511 North Trenton Street, Ruston, LA ZIP. If Prize winner is unable to come to that location to claim the Prize, he or she may make alternate arrangements if available, with an official at Community Trust Bank, to claim the prize within the ten (10) business day period described above.
Requirements of the Prize Winner: The Prize winner will be required to sign an Affidavit of Eligibility and Release of Liability for Participation, Publicity and Use (the “Affidavit”) as a part of determining the Prize winner’s eligibility and, prior to, and as the condition of, his or her acceptance and use of the Prize. If the
Affidavit and all required documents are not completed as specified, are not received by Community Trust Bank within fourteen (14) days of notification, or after Community Trust Bank’s receipt of the Affidavit from the Prize winner, the Prize winner is found to be an ineligible entrant, the entry will be disqualified and an alternate Prize winner, randomly selected from the remaining eligible entries, at Community Trust Bank’s sole discretion, from among all remaining eligible entries will be contacted.
To Receive Prize Winner’s Name or the Official Rules: To request the name of the Prize winner, available thirty (30) days after the end of the Promotion period, or a copy of these Official Rules, mail your request, along with a self-addressed stamped envelope to: Community Trust Bank eStatement Promotion, 1511 North Trenton Street, Ruston LA, ZIP. Please specify whether you are requesting the name of the Prize winner or a copy of the Official Rules. Requests received more than ninety (90) days after the end of the Promotion Period will not be honored.
Disputes: Each entrant agrees that: (i) any and all disputes, claims, and causes of action arising out of or connected with this Promotion shall be resolved individually, without resort to any form of class action, and exclusively by the Federal and State courts, (ii) any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Promotion, but in no event attorneys’ fees, and (iii) under no circumstance will an entrant be permitted to obtain awards for and entrant hereby waives all rights to claim punitive, indirect, special, incidental and consequential damages and any other damages (other than for actual out-of-pocket expenses) and any and all rights to have damages multiplied or otherwise increased. Some jurisdictions do not allow the limitation or exclusion of liability for incidental or consequential damages, so the above may not apply. All issues and questions concerning construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the entrant and Community Trust Bank in connection with this Promotion, shall be governed by, and construed in accordance with, the laws of the state of Louisiana, without giving effect to any choice of law or conflict of law rules (whether of the State of Louisiana or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the State of Louisiana. All Louisiana state and local laws and regulations apply.
Indemnification and Release: Each entrant agrees to defend, indemnify and hold Community Trust bank, its affiliates, parent organizations, subsidiaries and divisions and their respective representatives, attorneys, assigns, agents and licensees and their respective affiliates, equity holders, officers, directors, managers, agents, co-branders, associated marketing firms, advertisers and sponsors or other partners, and any of their employees (each, including Community Trust Bank, a “Release and Indemnified Party,” collectively, including Community Trust Bank, the “Released and Indemnified Parties”), harmless from and against any and all rights, demands, losses, liabilities, damages, claims, causes of action, actions and suits of any kind whatsoever (no matter whether at law or equity), fees, costs and attorneys’ fees (including appellate level) whatsoever (each, a “Claim,” collectively, “Claims”), brought or asserted by any third party against any of the Released and Indemnified Parities arising out of or in connection, directly or indirectly, with the entrant’s entry in the Promotion, participation in the Promotion, the Promotion, the Prize, and/or use or misuse of the Prize.
By entering this Promotion, each entrant agrees to release, discharge and hold harmless each Released and Indemnified Party from and against any injury, loss, claim, cause of action, or damages arising out of or in connection with, indirectly or directly, entry or participation in this Promotion and acceptance, use, or misuse of the Prize, including without limitation, personal injury and/or death or property damage.
Limitations of Liability: Community Trust Bank is not responsible for (i) electronic, human, United States Postal Service, equipment programming or other error, omission, interruption, defect or delay which may occur in the administration of the Promotion or the processing of any entry, the Prize, in the Official Rules, or in any other materials related to the Promotion, (ii) technical failures of any kind, including, but not limited to, the malfunctioning of any electronic equipment, computer, Internet, website, network, hardware or software, a computer virus, bug or other technical problem, (iii) unauthorized human intervention in any part of the entry process or this Promotion, (iv) any entrant’s conduct in connection with this Promotion, (v) any printing, typographical, technical errors in any materials associated with the Promotion or the Prize, (vi) incorrect, inaccurate, late, lost, stolen, illegible, incomplete, or misdirected entries received through impermissible or illegitimate channels, or (vii) any death, injury, or damage to persons or property, which may be caused, directly or indirectly, in whole or in part, from an entrant’s entry or participation in this Promotion or use or misuse of the Prize.
